WWLHIN Rural Health Partner Survey

The WWLHIN, in partnership with its health service providers, is undertaking a review of Community Support Services in rural Waterloo/Wellington.

The purpose of the review is to identify gaps in Community Support Services which are needed to provide appropriate assistance for people to return from hospital and to remain safely and independently in their home.

Waterloo/Wellington service providers/partners in rural health care are encouraged to offer feedback in a short survey which can be accessed here.

The survey will take approximately 10-15 minutes of your time, and will provide invaluable input into the review.

The results of the review will be presented to the Waterloo Wellington Rural Health Network, along with recommended next steps.

Please try and complete this short survey by April 11, 2012
